Caring for our growing population of elderly people living in care homes is one of the biggest challenges currently facing the NHS. Sutton Clinical Commissioning Group is now leading the way nationally on improving care in residential and nursing homes through the vanguard site Sutton Homes of Care, part of the vision of the NHS’s five year forward view.
On 26 October 2015, Sutton Homes of Care launches its pilot project under the vanguard programme: six nursing homes in the London Borough of Sutton who will be delivering a number of exciting new initiatives to improve care for elderly residents.
The launch will consist of a presentation by Dr Chris Elliot, Clinical Chair of Sutton CCG and a local GP, followed by lunch. We would be delighted if you could join us to help us celebrate this exciting development.
The pilot scheme will introduce a number of improvements to care for elderly residents, including:
· An improved system of care home reviews to maintain excellent quality of care and identify potential issues before they arise
· A programme of training and skill development for care home staff to create an environment of improved knowledge and skills alongside tools to support staff to make better care decisions for their residents
· A new role of Care Co-ordinator in each nursing home, who will work as part of a multi-disciplinary team to ensure continuity of care
