The event will take place at the Belmont centre run by Sutton Mental Hleath Foundation on tuesday September 22 from 1.30pm to 4pm.
"An event to explore the impact of and link between long-term health conditions e.g. diabetes, coronary heart disease, COPD etc. and mental and emotional health and to co-create improved self-management tools," said a spokesperson.
"The discussion will be led by Dr Chris Keers from Sutton Clinical Commissioning Group.
What are long-term health conditions?
"How do these conditions impact on people’s mental and emotional health? What can be done to improve people’s health and wellbeing? How are self-management skills of benefit to recovery?"
