Hospital Pathfinder volunteer
The initiative from Sutton Age UK allows the hospital Pathfinder volunteer helps to provide a support service to older people in Sutton following a hospital admission, especially to those who have little or no friends/family support network by connecting them to local community and support services, and by providing social and emotional support immediately after discharge.
The summary of duties:
•Make wellbeing phone calls to individuals after their discharge from hospital
•Work in the community to connect older people to voluntary services
•Provide administration support for the service
•Help older people to rebuild their confidence following a hospital stay
•Promote voluntary services on hospital wards
Anyone interested would have a desire to make a difference in older people’s lives. You would be a compassionate individual and able to make a time commitment of at least 2 hours per week. You should be willing to work in a hospital environment and carry out home visits if necessary. Good communication skills are essential and a good telephone manner is highly desirable. You do not have to have had previous experience in a hospital environment, although this would be helpful. Car driver is desirable, but not essential. This volunteer role would be ideal for individuals seeking a career or who want to be involved in the fields of health and social care.
